Initial Comments:
An onsite follow-up survey completed on November 14, 2023 found that St. Luke's Bethlehem Dialysis had corrected the following condition level and standard level deficiencies cited under 42 CFR, Part 494, Subparts A, B, C, and D, Conditions for Coverage of Suppliers of End-Stage Renal Disease (ESRD) Services. The deficiencies were cited as a result of a Medicare recertification survey that was completed on October 5, 2023.
